Carcinoma of the oropharynx represents an entity whose management is controversial and varied. Though some tumors arising in Waldeyer''s ring are chemo and radiosensitive, surgical management of many tumors represents an effective treatment. Today, many patients are being treated with chemotherapy and irradiation and the art of surgical management in this area is being lost and less-emphasized in several training centers. Many surgical approaches to access and resect these tumors will be presented in this course. A brief review of reconstructive options and effects on swallowing, speech airway, and aspiration will include the treatment of soft palatal, tonsillar, tongue base, and hypopharyngeal tumors. Options include endoscopic laser resections with indications, advantages, and disadvantages of each approach. Mandibular split, transhyoid, lateral pharyngotomy, and cold knife resections will be discussed. The presenters'' experience will be presented with a chance for interactive audience participation to aid in understanding the consequences of resection of each type of tumor.
